Physical Therapy Assistant

Job Summary: The Rehab Assistant is responsible for age appropriate interventions for the adolescent, adult, and older adult client. These interventions include meeting the physical, emotional, developmental, psychosocial, and educations needs of the client who requires rehabilitative and/or vocational care. The Rehab Assistant is responsible to collaborate with all disciplines throughout the episode of care, to ensure all goals are met. The Assistant provides delivery of high quality continuous care providing therapeutic exercise, education, treatments, and maintenance therapy in the patient's home. These services are provided under the supervision of a Physical Therapist or Occupational Therapist and in accordance with the physician's orders and as defined by generally accepted Physical Therapy or Occupational Therapy standards. Core Responsibilities and Essential Functions: Direct Patient Care - Access patients rehabilitative needs utilizing general principles governing reasonable and necessary therapy. - Schedule visits per MDs order. Adjust frequency as needed based on status and MD order - Perform routine physical therapy procedures to meet the need of the patient. - Meet established productivity standards. - Meet with all disciplines to review all cases being treated and to subsequently document this communication Coordination of Services - Participates in the development, implementation, and revision of the patients plan of care to ensure quality and continuity of care and proper discharge planning - Coordinate the care and services with the Therapists, Managers and other team members and support services. - Perform other duties as assigned by Home Health Leadership. - Maintain communication with MD reporting any changes that may affect the patients recovery. Patient/Caregiver Education - Serve as patient educator to foster care or caregiver support for the home patient. - Be aware and report concerns to authorities or agency leadership on any issues that affect patient or staff safety - Utilize both verbal and written tools to facilitate learning and promotion of independence during recovery. Staff Education and Professional Development - Responsible for attending continuing education and in-services as directed and needed - Accept responsibility to remain current in clinical knowledge for the care of the home health patient Communication and Documentation - Responsible for accurate completion of skilled intervention documentation within established timeframe. - Demonstrate knowledge and appropriate application of regulatory guidelines - Participate in care team conferences with coworkers - Responsible for checking voice mail box and secure messaging a minimum of once daily. REGULATORY GUIDELINES - Demonstrates knowledge and appropriate application of CMS/State, and Joint Commission regulatory guidelines. - Responsible for accurate completion of required visit documentation in the patients EMR - Completes all documentation within established timeframes Required Minimum Education: Graduate of an accredited physical therapy assistant or occupational therapy assistant program. Required Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated.

Additional License(s) and Certification(s):Required Minimum Experience: Minimum 1 year of Full-time Physical Therapy or Occupational Therapy Assistant experience Required and Home Health experience Preferred Required Minimum Skills: Problem solving skills, strong organizational and time management skills. Strong assessment and rehabilitative skills. Strong written and verbal communication skills, Ability to write and follow directions to clients homes. Use of window based computer, utilizing home health patient software, email, internet, computer based learning modules, etc. Good understanding of home health operations, reimbursement, law and regulatory processes.
