Core Responsibilities and Essential Functions:

Patient Assessment

  • Collaborate with physicians to establish and evaluate treatment programs, care plans, goals, and discharge plans.
  • Develop a care plan with input from the patient and/or family.
  • Create realistic discharge plans that align with patient and family goals.

Treatment Planning and Provision of Care

  • Implement the physical therapy treatment plan.
  • Select appropriate treatment activities to advance patient goals.
  • Revise goals and care plans with patient/family input.
  • Provide ongoing patient and family education and training.
  • Offer discharge instructions and referrals to community resources as needed.
  • Serve as a referral source for rehabilitation services and equipment.

Documentation

  • Document evaluations, care plans, and treatment sessions according to departmental guidelines.
  • Complete and submit all required documentation, including family education and discharge information.

Required Minimum Education:

  • Graduate from an accredited physical therapy program.
  • Bachelor’s, Master’s, or Doctorate in Physical Therapy.

Required Minimum License(s) and Certification(s):

  • Physical Therapist License.
  • Basic Life Support (BLS) Certification.
  • BLS Instructor Certification (if applicable).
  • BLS Provisional Certification (if applicable).

Required Minimum Skills:

  • Effective communication, customer service focus, team orientation, and basic computer skills.
