Core Responsibilities and Essential Functions:
Treatment Planning and Patient Care
- Implements occupational therapy treatment plans.
- Selects appropriate treatment activities to help patients achieve their goals.
- Revises goals and treatment plans with patient and family input.
- Provides ongoing education and training to patients and families.
- Offers discharge instructions, follow-up guidance, and referrals to community resources as needed.
- Serves as a resource for staff, families, and physicians regarding rehabilitation services and equipment.
Patient Assessment
- Works with physicians to establish and continuously evaluate treatment programs, care plans, goals, and discharge plans.
- Develops a plan of care in collaboration with the patient and/or family.
- Creates a realistic discharge plan aligned with patient and family goals.
Documentation
- Documents evaluations and care plans in accordance with departmental guidelines.
- Records each treatment session following departmental protocols.
- Completes and submits required documentation, including patient education and discharge summaries, per facility and program guidelines.

Qualifications:
Education:
- Master’s degree in Occupational Therapy or Associate’s degree in Occupational Therapy (required).
Licenses & Certifications:
(All certifications must be obtained upon hire unless otherwise specified.)
- Occupational Therapist License (required).
- Basic Life Support (BLS) certification from AHA (required).
Experience:
- Minimum of three (3) years as an Occupational Therapist (required).
Skills:
- Effective communication skills.
- Strong customer service orientation.
- Ability to work collaboratively in a team environment.
- Basic computer proficiency.
