Job Summary:
Wellstar Health System is seeking a compassionate and dedicated healthcare provider to join our extended hours model, offering alternative primary care visit times to meet the needs of our patients at East Cobb Health Park. The ideal candidate will deliver high-quality care by providing flexible, accessible, and patient-centered services outside traditional office hours. As a provider in this innovative care model, you will meet patients where they are, ensuring they receive timely care that fits their busy schedules.

Key Responsibilities:

• Provide comprehensive primary care services to patients during extended hours, including some evenings and weekends.

• Conduct thorough assessments, diagnoses, and treatment plans for patients across various age groups and medical conditions.

• Offer an alternative to traditional in-office visits, ensuring patients can access care at times that work best for them.

• Collaborate with multidisciplinary teams to coordinate care and follow-up as necessary, ensuring continuity of care.

• Use EPIC to document patient interactions, treatment plans, and outcomes in a timely and accurate manner.

• Provide patient education, health counseling, and resources to support patients' overall well-being and health goals.

• Adapt to the diverse needs of patients, demonstrating flexibility and empathy while delivering high-quality care.

• Participate in continuous learning and stay up-to-date with best practices in primary care medicine.

• Maintain compliance with Wellstar Health System's policies, procedures, and regulatory standards.

Preferred Qualifications:

• Experience working in an extended hours or urgent care setting.

• Bilingual (Spanish/English) preferred but not required

Requirements: 

• Active or eligible for Georgia NP or PA License/Certification
