As a Care Partner, you will work under the guidance of a Registered Nurse to enhance patient experiences and provide compassionate care. Your key responsibilities include assisting with daily living activities (ADLs), promoting patient comfort, and collaborating with healthcare professionals. Thriving in a fast-paced environment and effective communication with the healthcare team are essential for success.
Core Responsibilities and Essential Functions:
Patient Experience: Collaborate with patients, families, and the healthcare team to create a healing environment. Address patient needs and engage them in their treatment plans.
Patient Care: Provide hands-on care, including hygiene, nutrition, and comfort measures. Assist with advanced skills as required by the unit.
Teamwork: Support unit initiatives and participate in performance improvement efforts. Transport supplies and medications as needed.
Communication: Utilize effective communication techniques, ensuring clarity with patients and colleagues. Document patient care accurately in the Electronic Medical Record (EMR).
Safety & Infection Control: Maintain a safe, clean environment, adhere to infection control protocols, and reinforce the importance of universal precautions.
Required Qualifications:
Education:
·High school diploma or equivalent preferred.
Licenses/Certifications:
·Basic Life Support (BLS) certification is required.
·Certified Nursing Assistant certification is preferred.
Skills:
·Strong interpersonal abilities, clear communication skills, and the capability to follow detailed instructions.
·You should be comfortable working both independently and as part of a team.
