The Sup RN Clinical HH is a proactive member of an interdisciplinary team of licensed and unlicensed care givers who ensure that patients, families and significant others receive individualized high quality, safe patient care. Job Summary: The nurse supervisor of the high risk home health program is a registered professional nurse who provides leadership to a specialized group of clinicians, through the effective use of home health knowledge and home care nursing skills to create a center of excellence that promotes the mission and vision of WellStar Home Health. The supervisor works with the manager nurse home health, coordinating team members, and maintaining quality of care as well as acting as a communication resource between the agency, staff, physicians and other entities . He/She fosters an environment of trust by open communication, promoting teamwork, inclusion and respect. The RN Supervisor Role specific competencies, responsibilities, roles and functions are based on the Professional Practice Model, 5-Star Nursing. It is expected that all RN Clinical Nurses are licensed, knowledgeable and uphold the practice of nursing as outlined by the Georgia Professional Nurse Practice Act and implements the Scope of Practice and Code of Ethics Standards put forth by the American Nurses Association. As a member of the patient services team, it is expected the individual upholds the voice of the patient, system policies and procedures while supporting service excellence goals. Core Responsibilities and Essential Functions: People - Supervise direct reports and ensure productivity management expectations are met - Responsible for employee engagement, department retention, and turnover - Perform joint visits in the established timeframe and ensure training and development of staff is complete - Complete mid-year and yearly evaluations on direct reports - Work with Home Health Manager on necessary corrective action and to resolve complex issues Quality and Patient Safety - Work with the clinical home health team to ensure high risk patients on identied, assessed and receive proper and timely treatment - Ensure regulatory requirements are met - Accountable for compliance with any regulatory or accreditation standards as appropriate - Collaborate with system initiatives, to reduce hospitalizations and decrease the length of stay - Ensure patient clinical outcomes through HHCAHPS CMS Star ratings Patient Experience - Serves as a communication resource between Wellstar Home Health, patients, physicians, and other entities - Ensures effective communication with other Wellstar departments and team members - Serves as resource to perform any service recovery as necessary Financial - Works in conjunction with the scheduling team to ensure continuity of care and established productivity expectations are being met - Monitors the volume of patient turnaways daily to determine productivity management and the need for additional resources - Ensure Discharges are completed timely - Works with the Home Health Manager is ensure cost control initiatives are being met Successful Growth - Works in conjunction with Home Health Manager and Executive Director regarding strategic planning and process improvement - Support the Home Health target growth budget - Recommends and implements any new programs or services for department Required Minimum Education: Graduate of an accredited school of nursing Required and Bachelor's Degree Preferred Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated.
  • Reg Nurse (Single State) or RN - Multi-state Compact
  • Basic Life Support or BLS - Instructor
  • Georgia Driver's License
Additional License(s) and Certification(s): Licensed as a registered nurse in the state of GA Upon Hire Required Required Minimum Experience: Minimum 1 year in the Medical/surgical setting Required and Minimum 1 year successful supervisory and Homecare Experience Required Required Minimum Skills: Should possess excellent verbal and written communication skills and relationship building; a knowledge of the health care environment; leadership that includes such skills as successfully implementing and managing a change process ability to use systems thinking- and inspire, coach others to be successful; model the way for professional practice; professionalism and teamwork/collaboration for self/department/medical staff; and possess necessary business skills to manage human and material resources. Must be a continuous learner who understands health care financing; strategy and operations for running a business unit(s); and effectively employ data and technology to support work processes and make decisions. Supports shared governance or shared decision making; manage the design and delivery of care that in based on evidence and focused on quality and safety. Knowledge of risk management and various accreditation/regulations/licensing requirements assuring that these are upheld while advancing industry standards through the achievement of goals by effective management and measurement of outcomes.
