Job Summary: The Social worker is to provide assistance and guidance to residents, families and staff in the formulation of plans for continuity of care following the admission to the nursing home and /or following discharge from the LTC facility. Core Responsibilities and Essential Functions: Essential Duties and Responsibilities - Possess the ability to interview residents and family members to gain knowledge concerning both social and psychological needs. - Demonstrates ability to gain rapport and confidence of the resident and family. - Exhibits caring attitude that is reflected in response of residents, families and personnel. - Screens admissions and formulates plans for post nursing home care. - Provides support to families coping with nursing home placement. - Maintains a current knowledge base about community resources to help meet the individual's psychosocial needs. - Acts as a liaison between the resident, their families, personnel and outside institutions. - Participates in the resident education process. - Ensures professional growth through keeping up-to-date on current medical social work trends. - Documentation is current, clearly written and concise. - Other duties, as assigned. Regulatory Responsibilities - Participates in inter/intra departmental team activities to improve process and patients outcomes. - Effectively communicates with internal/external customers and treats them with respect. - Maintains a good working relationship with internal/external customers. - Practice positive customer service skills. - Utilizes proper telephone etiquette. - Adheres to hospital wide and departmental policies and procedures. - Identifies improvements that can be made in job performance. - Adheres to and abides by HIPPA and the West Georgia Health System Compliance Program. - Complies with Annual Health Assessment requirements. - Completes all mandatory education requirements. Required Minimum Education: Bachelor's Degree in Social Work Required or related field, preferably from a university accredited by the Council on Social Work Education. Required and Master's Degree Preferred Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated.
  • Georgia Driver's License
  • Basic Life Support or BLS - Instructor
Additional License(s) and Certification(s):Required Minimum Experience: Minimum 1 year of experience in a skilled care environment Required and A background in medical social work and/or work experience in other areas of social work Preferred Required Minimum Skills: Basic typing skills & excellent communication skills. Knowledgeable regarding local, state and federal resources and programs. Ability to function well under stress. Self-directed. Ability to communicate effectively with others. Must possess maturity, self-confidence, objectivity, and positive attitude. Ability to empathize and handle stress associated with this particular position.
