Job Summary:

The Pulmonary Disease Coordinator, functions under the direction of the RC Manager/Director and is responsible for providing age-appropriate pulmonary disease education and management addressing the biological, emotional, and developmental, psychosocial needs of the patient/family within the acute care setting all while performing routine respiratory patient care activities. The Pulmonary Disease Coordinator collaborates with care coordination for discharge readiness and functions as the liaison and /or facilitator for patient and families between other inpatient disciplines, outpatient services and providers as it relates to improving patient self-management skills, medication access and adherence and disease knowledge. May assist with new program development including but not limiting to community /population health initiatives. Role is responsible for data collection, analysis and dissemination of disease specific outcomes based on best practice and evidence-based therapy. The Pulmonary Disease Coordinator may also administer medication and implement respiratory care plans as prescribed by provider and within the scope of their Respiratory Care Professional state license.

Core Responsibilities and Essential Functions:

Patient and Family Teaching Provide teaching according to diagnosis, treatment plan and patients learning needs to include but not limited to: Normal lung anatomy & physiology, Physiologic changes caused by disease process, Medication classifications, administration and proper technique, Action Plan and Zones, recognizing exacerbation, breathing exercises and stress control, smoking cessation, oxygen use Provide teaching to patient, patient family or caregivers Provide excellent customer service for patients, families, providers and other health care professionals Administers Quality Patient Care Performs patient assessment & assessment for therapy Collaborates with health care team members in planning patient care Verifies orders and administers aerosolized medication, bronchial hygiene, volume expansion, oxygen & medical gas therapy Performs ventilator management, weaning, wave form graphic interpretation; airway management (intubation, extubation, trach tube change and care) Performs arterial blood gas sampling & analysis Educates patient and mentors/precepts new employees, students and peers Location Specific: Assisting in invasive and non-invasive procedures, Cardiac Service procedures, Pulmonary Diagnostic Testing Team Responsibilities Provide a team centered approach of communication regarding teaching and instruction plans Serve as a mentor to staff, students and peers Support performance improvement initiatives designed to improve promote quality of patient care across the continuum of care Maintain flexibility to respond to changing needs of team Support fiscal accountability for human and material resources Collaborate with leadership to promote positive team atmosphere Teaching Resources Utilize approved written education materials along with educational aides Assist with material assimilation (ordering and preparing folders) Assist with material updates to reflect changes in pulmonary scope of practice Facilitate Continuum of Care Process Participate in multidisciplinary rounding (MDR) for chronic pulmonary patients Evaluate patient home plan and coordinate adjustments with care team for medication administration, oxygen therapy, home monitoring and durable medical equipment as it pertains to current pulmonary condition Provide patient and families with community disease management programs. Performs other duties as assigned Complies with all Wellstar Health System policies, standards of work, and code of conduct.

Required Minimum Education:

  • Bachelors Respiratory Therapy

Required Minimum License(s) and Certification(s):

All certifications are required upon hire unless otherwise stated.
  • Respiratory Care Prof
  • Basic Life Support or BLS - Instructor-Preferred
  • Advanced Cardiac Life Support or ACLS - Provisional or ACLS - Instructor-Preferred
  • PALS - Provisional within 180 Days or Pediatric Adv Life Support within 180 Days or PALS - Instructor-Preferred within 180 Days
  • Neonatal Resuscitation Prvdr-Preferred within 180 Days or NRP-Provisional-Preferred within 180 Days or NRP - Instructor-Preferred within 180 Days

Additional License(s) and Certification(s):

Required Minimum Experience:

Minimum 4 years comprehensive experience in Respiratory Care Required and Minimum 2 years Two-year experience with clinical teaching Preferred

Required Minimum Skills:

Ability to communicate well, critical thinking skills, establish and maintain working relations with physicians and other health care personnel, and computer skills including familiarity with databases. Good customer relation, assessment, and communication skills are a must for this job.
