Job Summary

The Physical Therapist assesses, plans, organizes, and participates in rehabilitative programs to enhance mobility, relieve pain, increase strength, and prevent or reduce deformities in patients with diseases or injuries. Services are provided throughout the care continuum with a focus on cost-effectiveness, quality, and customer satisfaction. Compliance with all relevant regulatory standards is required.

Core Responsibilities and Essential Functions:

Patient Assessment

  • Collaborate with physicians to establish and evaluate treatment programs, care plans, goals, and discharge plans based on patients' physical therapy needs.

  • Develop a care plan with input from the patient and/or family members.

  • Create realistic discharge plans that incorporate patient and/or family goals.

Treatment Planning and Provision of Care

  • Implement the physical therapy treatment plan.

  • Select appropriate treatment activities to help patients achieve their goals.

  • Revise goals and care plans based on feedback from patients and families.

  • Provide ongoing education and training to patients and families.

  • Offer discharge instructions, follow-up, and referrals to community resources as needed.

  • Serve as a referral source for staff, families, and physicians regarding rehabilitation services and equipment.

Documentation

  • Document evaluations and care plans according to departmental guidelines.

  • Record each treatment session following departmental protocols.

  • Complete and submit family education, discharge, and other required documentation in line with facility and program guidelines.

Required Minimum Education

  • Graduation from an accredited physical therapy program.

  • Bachelor’s, Master’s, or Doctorate degree in Physical Therapy.

Required Minimum Licenses and Certifications

  • Physical Therapist license.

  • Basic Life Support (BLS) certification.

  • BLS Instructor certification (if required).

  • BLS Provisional certification (if required).

Required Minimum Skills

  • Effective communication skills.

  • Customer service-focused.

  • Team-oriented.

  • Basic computer skills.
