Wellstar Medical Group is seeking a Nurse Practitoner or Physician Assistant to work alongside a collabortive Transplant Surgery team at Wellstar Medical College of Georgia in Augusta, GA.

The APP will provide focused inpatient and outpatient care to the transplant surgical patient population. Review all referrals and determine initial plan of care to optimize clinical visit. Perform history and physical examinations on surgical patients; evaluate new and established patients Assist with patient needs (assess hernias, wounds, vascular access, etc) in the transplant clinics / inpatient as needed Perform consultations and present appropriate information to Attending Surgeon Acquire/analyze patient care data and information, then present findings/recommendations to attending surgeon Schedule patients for diagnostic tests, operations and coordinate patient visits for other required services such as Interventional Radiology for care of vascular access Provide patient education and family counseling regarding health care plans and procedures Follow up on diagnostic testing results, post-operative issues, pain management and lab results for both inpatients and outpatients Triage and manage patient calls into the transplant surgery service.  Assess urgency of issues and implement plan to handle emergent problems. Act as the regional source for information and patient education for transplant services. Assist with outreach clinics as needed

Procedural

Placement and removal of drains and catheters as directed by attending Surgeon (includes foley catheters and JP drains) Removal of sutures and staples, and performs complex dressing changes up to and including wound vac Instruct patients on wound care and stoma care Manage the sterile tray process needed for removal of catheters in the clinic.

Communication

Respond to telephone calls and provide a triage service to patients and families Establish a formal referral process for all outlying hospitals in the region and areas served by our Satellite Clinic System for patient transfers and consultation Coordinate regular meetings and conferences for the section Relay information to referring physician offices as needed (pathology reports, discharge summaries, CT scan reports, etc.) Maintain open communication with dialysis units regarding patient access needs and follow up Report patient or referring physician issues to transplant attending surgeons as needed. Advocates for patient needs to the attending surgeon and ancillary services.

Documentation

Prepares and maintains accurate patient records and documentation to support sound medical practice and reimbursement for services provided. Complete required paperwork for scheduling surgeries Write admission orders, peri-operative orders and discharge orders Record progress notes, procedure notes, and brief operative notes Appropriate prescription privileges in accordance with DEA regulations and Georgia state laws Perform dictations and medical record documentation during and after clinics Keep a database of patients and their anticipated needs.
