Data Integrity Chart Correction Analyst

Job Summary:

Monitor the Chart Correction Help Desk, research, open and assign cases and/or tasks to assure accurate and timely chart corrections. Oversee chart corrections of all patient data systems and related records to include Epic, all third-party systems, and retired platforms maintained in the Data Repository. Assist end users with corrections. Ensure synchronization of corrections in third party systems, integrated applications, and shared data. Monitor open cases to ensure accuracy and timeliness of resolution

Core Responsibilities and Essential Functions:

1. Customer Focus/Job Knowledge a. Monitor the Chart Correction Help Desk. Review and open chart correction cases per Chart Correction Help Desk established procedure and assign cases and/or tasks to assure accurate and timely chart corrections. b. Research each chart correction error to identify complexity of case, including integration of third party systems, downstream systems, interface messages, and shared data links via Care Everywhere to determine root cause analysis and detailed plan for resolution. c. Identify and document the location of the error, end user responsible, all applications impacted, shared data via Care Everywhere and Epic Care Link, coding and billing impact, MyChart impact and others as needed. Create tasks for each end user with detailed instructions on where and how to make their corrections. d. Assist end users with correction including but not limited to merge errors, overlays, charting errors, documentation errors, registration errors, billing and/or guarantor errors, scanning errors, amendment requests, Medical Identity Theft, interface error messages, and/or any identified Chart Correction issues, performing contact moves as needed using the Chart Correction Workspace e. Ensure synchronization of corrections in third party systems, integrated applications, Legacy Data Repository, and shared data including but not limited to Care Everywhere, EpiCare Link, GRITS, SSA, and any other shared links f. Monitor and resolve open chart correction cases in a timely manner and according to established chart correction procedure g. Assist with Medical Identity Theft resolution according to established policy and procedures and in conjunction with Compliance h. Assist with Amendment Request in compliance with established policy and procedure i. Work with Connect analyst to test and validate Epic upgrades, system updates, new functionality, testing and validation of workflows, and application issues as needed. j. Assist Manager and Supervisor with various reports, statistics, and communications as needed. 2. Customer Service a. Assist end users with education and assistance making chart corrections to ensure accuracy, timeliness, and integrity, making sure they have the access needed to make the correction, detailed instructions, and the time parameters in which it can be made. b. Respond to chart correction phone calls and general e-mail/inbox questions in a timely manner c. Create and provide educational materials for training and awareness. d. Talk end users through how to prevent errors in the future, and impact of errors for future reference. Required Minimum Education:

  • High School Diploma Accounting or GED Accounting or Accredited Program Health Information Management

Required Minimum License(s) and Certification(s):

All certifications are required upon hire unless otherwise stated.

    Required Minimum Experience:

    Minimum 3 years combined experience working in a Health Information Management, EMR, IT, or clinical role Required and experience correcting medical records and/or experience in patient registration systems and/or clinical workflows. Required Graduate of a health information technology (HIT) Program or clinical certification or department established equivalent. Required

    Required Minimum Skills:

    Required to have working knowledge of Word, Excel, Power Point and Outlook Must have excellent analytical and computer skills (tech-savy) Must have excellent communication skills to be able to communicate with end users effectively, as well as with various levels of staff within organization (physicians, clinicians, leadership) Strong critical thinking skills to determine root cause for data integrity issues. Ability to analyze and troubleshoot problems and offer solutions.
