The Physical Therapist assesses, plans, organizes, and participates in rehabilitative programs to enhance mobility, relieve pain, increase strength, and prevent or reduce deformity in patients with diseases or injuries. Services are provided throughout the care continuum in a cost-effective, high-quality, and customer-focused manner. Compliance with all relevant regulatory standards is required.
Responsibilities:
Patient Assessment
Collaborate with physicians to establish and continually evaluate treatment programs, care plans, goals, and discharge plans tailored to the patient's physical therapy needs.
Develop a care plan with input from the patient and/or family.
Create a realistic discharge plan incorporating patient and/or family goals.
Treatment Planning and Provision of Care
Implement the physical therapy treatment plan.
Choose appropriate treatment activities to help patients achieve their goals.
Adjust goals and care plans based on patient and family feedback.
Provide ongoing education and training to patients and families.
Offer discharge instructions, follow-up, and referrals to community resources as needed.
Serve as a referral source for staff, families, and physicians regarding rehabilitation services and equipment.
Documentation
Document evaluations and care plans according to departmental guidelines.
Record each treatment session per departmental protocols.
Complete and submit family education, discharge, and other required documentation within facility and program guidelines.

Qualifications:
Required Minimum Education
Graduation from an accredited physical therapy program.
Bachelor's, Master's, or Doctorate degree in Physical Therapy.
Required Minimum Skills
Effective communication skills.

Basic computer skills.
Required Minimum License(s) and Certification(s)
Physical Therapist license.
Basic Life Support (BLS) certification.
BLS Instructor certification (if required).
BLS Provisional certification (if required).
