Facility: Wellstar Primary Care - Hiram

Wellstar Health System is seeking acompassionate and skilled Advanced Practice Provider (APP)to join ourGeneral Cardiologyteam atWellstar Cardiology Care in Hiram, Georgia. The APP will work collaboratively with our cardiologists and the interdisciplinary team to provide high-quality, evidence-based cardiovascular care to adult patients.

Responsibilities:

• Conduct comprehensive cardiovascular assessments and physical exams.

• Manage follow-up and new patient visits under the supervision of cardiologists.

• Monitor and manage common cardiac conditions such as hypertension, heart failure, arrhythmias, and coronary artery disease.

• Provide patient education regarding disease processes, medications, and lifestyle modifications.

• Perform medication reconciliation and titration of cardiac medications.

• Participate in care coordination with referring physicians and specialists.

• Document patient encounters and treatment plans accurately and timely in the EMR (Epic).

Qualifications:

• Nurse Practitioner (NP): Master’s or Doctorate of Nursing Practice with current RN and NP licensure in Georgia.

• Physician Assistant (PA): Graduate of an accredited PA program with current licensure in Georgia.

• Certification by appropriate national board (AANP, ANCC, or NCCPA).

• Active BLS and ACLS certification required.

• Minimum 1-2 years of cardiology or internal medicine experience preferred; new grads with strong clinical rotations in cardiology will be considered.

• Excellent communication, organizational, and time management skills.

• Ability to work independently and collaboratively in a fast-paced environment.

Schedule:

• Full-time, Monday–Friday daytime hours

• 4/10's
